Women randomized to the New Nordic Diet meet the study dietician for 1.5 hr of individual diet treatment according to the New Nordic Diet and a cognitive behavioral approach. The diet advice include evenly distributed meals over the day, foods low in fat and rich in fibre, 500 g fruit and vegetables daily, fish 2-3 times a week and keyhole foods.

The control women receive diet advice according to usual care.

Gestational Diabetes

The New Nordic Diet conforms with the Nordic Nutrition Recommendations and incorporates environmental sustainability and planetary health. It includes rapeseed oil, whole grain bread, wild fish, seafood and game, potatoes, root vegetables, cabbages, Nordic fruits and berries. Women will be given advice on this diet by registered dietician. Every second wk, women are contacted through text messages to send blood glucose values and receive feedback.( All Swedish women developing gestational diabetes receive HemoCue within usual care for blood glucose monitoring.)

The control women receive diet advice according to usual care, which may be a short meeting with the regular midwife or a short meeting with a dietician.

Outcome is HbA1c at week 37 of gestation and lab personnel are masked to treatment allocation. Initial data reaching the Investigators will not have the code broken.

Ultrasounds of fetal growth will be performed and after birth anthropometrics of the newborn will be meaured including a DXA-scan to estimate bodycomposition.

20 pregnant women matched on age, prepregnancy-BMI and parity (n = 20). During pregnancy the women will be tested with mixed meal test and continuous glucose monitoring for the diagnosis of hypoglycemia. Ultrasounds of fetal growth will be performed and after birth anthropometrics of the newborn will be meaured including a DXA-scan to estimate bodycomposition.

Pregnancy

Obese pregnant women from PHCC randomized to this group will receive Home-based Diet & Physical activity (PA) counseling. Home-based intervention will consider 2 home visits of one hour of duration. During this session dietary educational and behavioral will be discussed with the participants and their families tailoring the contents based on the specific contexts.

Lifestyle counseling

Obese pregnant women from PHCC randomized to this group will receive n3LC-PUFAs oral supplementation based on Schizochytrium oil (S-oil) containing 800 mg docosahexaenoic (DHA) acid/day, which will be administered as capsular preparations containing 200 mg DHA/capsule (4 capsules/day).

PUFA Supplementation

Obese pregnant women from PHCC randomized to this group will receive will receive routine diet & PA counseling (i.e. they will receive the standard education sessions included in the prenatal controls at PHCC but no home-based counseling sessions).

Routine diet & PA

Obese pregnant women from PHCC randomized to this group will receive capsular preparations containing 50 mg DHA/capsule (4 capsules/day), given that evidence suggests that it would be unethical not to provide DHA during pregnancy. Placebo will be delivered in the same way that the n3LC-PUFA supplementation.
