A case of a 31-year-old male was noted to have marked joint swelling and tenderness accompanied by elevated alkaline phosphatase, and florid periostitis on radiographs. The patient was under treatment for fungal infection with voriconazole. Discontinuation of voriconazole resulted in resolution of his symptoms and radiographic findings. Mechanisms for how voriconazole causes periostitis, including information relating to its pharmacologic characteristics and its effects on bone metabolism, are discussed. Increasing our knowledge on voriconazole-related periostitis have the potential to further our understanding of common disorders such as osteoporosis and tap into other therapeutic options for it.
